New Patient Information

I grant Cincinnati Family Vet, its representatives, and employees the right to take photographs of me and/or my pet, and to copyright, use, and publish the same in print and/or electronically. I agree that Cincinnati Family Vet may use such photographs of me and/or my pet with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and web content.

I authorize the veterinarian to examine, prescribe for, or treat the above-described pet(s). An estimate of care options will be discussed prior to treatment. I assume responsibility for all charges incurred in the care of this animal. I also understand that these charges must be paid in full at the time of the pet's release.
